  2. After you have watched the video, complete the form below and click "submit."  Your information will remain confidential.  (Note: It helps to print out your progress before you click "submit" in case there are any connection issues.)
  3. LifePoint will contact you with further instructions upon receipt of your Doorway Class form!

- A person must be born again to become a member of LifePoint.
- A person must be “scripturally” baptized to become a member of LifePoint. 
- A person must embrace “who LifePoint is” to become a member.  (see LifePoint Doorway Manual)
